If not already registered, we need: enrollment form, birth certificate or passport, immunization records, most recent physical/check-up, $25 registration fee.

Important Forms
You will need this account to check in at our facility. Then print one form per child. Please provide a birth certificate, shot, and physical record when registering for the first time. We will have to turn you away if you do not have the forms on file. To make it simple: Call your doctor and if your child has been to the doctors within the last 12 months, your doctor can fax over the forms to 540-548-1504.
